I posted this on a few other threads as well. So here you go. I understand you've probably tried some of these already, but bare with me, quite frankly, what do have to lose? I've had to fix this twice in the last week. The three things you need to do are download and install 'Games for Windows Live' found here V V and you also need to download and put in place these two files. One is a.dll file. The other is an altered FALLOUT.ini file.

V V I'm sure you've seen these fixes elsewhere and they might not have worked, my situation was the same, for some reason only this copy of the.dll actually worked for me, it might be the.ini that did the trick. Reguardless, just install all three of these, as you will need all three. Even after all of this, -tabbing out will still have a high chance of crashing the game-, so always save before you do!!
