If you are running XP, while using the built in tool, make sure that no other program runs to interrupt its task,otherwise it may hang or restart. If you are running Windows Vista, its designed to defrag on its own and doesnt show a graphic or fragmentation visuals. Its however very slow and runs in low priority in the background. Running the Disk Defragmenter can help take fragmented files and make them contiguous, thereby reducing the input/output operations when later accessing that file. It should b e understood that recording a file to disk to not ensure that it's recorded in one piece. In fact, frequently files are recorded in several pieces. In order to then access that file requires that each piece of that file be brought up seperately. The effect of which can slow performance and can lead to other problems in severe cases.

Disk defrag must be run regularly (or set to auto defrag) to keep the hard drive free of fragmentation and maintain the performance of the computer at a high level. Windows comes with a built-in defragmenter, but it's not very good IMO: it's very slow, doesn't defrag all files (especially important system files), doesn't defrag properly when disk space is low, and is not flexible. Combine this with the fact that the defragger doesn't even defrag all files properly, and the whole operation becomes a waste of time. However, third party defrag utilities, especially the advanced commercial utilities, that are specifically designed as automatic background defraggers have no such problems.
